The Senior LinkAge Line® is the Minnesota Board on Aging's free statewide information and assistance service. The Senior LinkAge Line® service is provided by six Area Agencies on Aging that cover all 87 counties of Minnesota and helps connect you to local services. 

The Senior LinkAge Line® is answered from 8 a.m. to 4:30 p.m. weekdays.

With a single call to 1-800-333-2433 you will get assistance with looking at your situation to determine what service might be helpful.

Senior Linkage Line® not only helps connect you to services in your community, but  also provides the actual assistance to help you get the information and personalized help you need to make good decisions. This includes providing face to face help to you in your home and community. The Senior LinkAge Line® has expertise in the areas of :

  • Prescription Drug Expense assistance for Minnesotans of all ages

  • Forms assistance, including help applying for Medical Assistance and Medicare options

Long Term Care Consultation Expansion

During the 2011 Special Session of the Legislature, a new initiative was adopted that requires the Senior LinkAge Line® and Lead Agencies (counties, Tribes to expand access to Long-term Care Consultation to those entering a Registered Housing with Service setting. Read the legislative language, (Subd. 3c Consultation for housing with services.).

Started October 1, 2011 all people interested in entering a registered housing with services setting, such as assisted living, must now be offered a long-term care consultation before signing a lease. 

Individuals calling will be screened for risks and if they are screened at high risk, will be offered a referral to the county team that provides long-term care consultation assessments.  Individuals will continue to choose their service provider and will have access to supportive planning for continued community living. For people who continue to transition to registered housing with services, additional resources for aging well and living well including access to caregiver supports and health insurance counseling.

  • The counties provide a MnCHOICES Assessment. These consultants can assist in getting access to the services you need to remain independent. Every Minnesotan, regardless of income, is entitled to a free Long-term Care Consultation.  www.minnesotahelp.info can help you find the county long-term care consultation program closest to you.

FREE SAFETY CHECK FOR SENIORS - 

HOUSEHOLD & OUTSIDE MAINTENANCE FOR ELDERLY (HOME)

No-cost Home Safety Check for senior citizens is offered by the Household & Outside Maintenance for Elderly (HOME) Program of Senior Community Services. We will check your need for handrails, grab-bars, smoke detectors, lighting, and other home safety components. Elders aged 60+ residing in suburban Hennepin County and Minneapolis are eligible for this limited time offer. No sales. Advent Lutheran Church is partnering with Osseo Senior High to provide a backpack program to the students. This is an amazing, ongoing outreach within our community for students who are facing food instability on the weekends and over holiday breaks. Each week this program supplies backpacks with non-perishable, easy to prepare food items. The students take home a backpack full of food on Friday after school and bring it back empty Monday morning. There are students in our community who are either homeless or do not know where or when their next meal is coming. Food donations may be dropped off by the Police Department door at Osseo City Hall during regular business hours.

Monetary donations may be mailed to Advent Lutheran Church, 9475 Jefferson Highway, Maple Grove, MN  55369. Please note all checks will need to include a memo indicating proceeds are for Backpack Program.
