Below are common forms that can be completed and returned to the Osseo Police Department.

First Person Statement

The First Person Statement should be used to give a detailed report regarding an incident that you have reported or have been a witness to.

First Person Statement Continuation

The First Person Statement Continuation form should be used if you need to give further details regarding an incident you have reported or have been witness to.

Vacation/Special Door Check

If you are going to be out of town and would like the police department to check on your house while you are away, just fill out the form and return it to the Osseo Police Department. While you are gone, we will log the date/time we checked your house and will notify you or the designated key holder of any problems we may find.

Citizens Arrest Report

The Citizens Arrest Report is to be used when you, the citizen, make a citizens arrest for a crime, or traffic violation you witnessed.

Business Registration Form

This form is used by the police department to contact key holders or business owners incase of an emergency. As key holder and owner information may change from time to time, please keep the police department up to date with those changes as well.
